Overview
Hayes Park Lodge is supported living accommodation for people with Mental Health issues and dual diagnosis. The purpose of the service is to assist/support and promote individuals to live independently in the community.
The service comprises of 20 flats – there are four buildings which house four individual flats – two bungalows containing flats and two flats situated above the main building over the staff office.
All flats are self contained with one bedroom, a separate bathroom , lounge and kitchen which includes a cooker, hob and fridge/freezer and washing machine .
There is plenty of parking via a gated entrance and a large communal garden with patio area.
A large communal lounge is in the main building for everyone to use.
The accommodation is within walking distance of the local town of Hayes and the main town of Uxbridge is a 10 minute bus ride. There are plenty of local shops and amenities and good public transport links.
Staff are on site 24 hours a day, 7 days a week to provide individually tailored support to the residents.
Hayes Park Lodge is a short term service of up to two years, for vulnerable adults with a mental health issue and dual diagnosis .
Ability staff support individuals to move on in to the community who can then be referred to our in-house floating support service.
What we offer

Ensure safety and wellbeing by working closely with specialist services to support individuals to work through their recovery.

The areas of support provided but not limited to:
- Daily living skills (cooking / cleaning)
- Budgeting, paying bills and preventing debt
- Applying for welfare benefits such as Housing Benefit, Council tax benefit, PIP
- Education, training, voluntary work and employment
- Health care appointments
- Leisure and culture
- Accessing the community
- Building relationships
- Repairs
- Health, safety and wellbeing
- Drug and alcohol support / signposting
Customer groups
Vulnerable adults with a severe and enduring mental health and dual diagnosis needs.
Referrals
All referrals come from the local Community Mental Health Teams.
All referrals need to be approved and agreed via the Mental Health Panel.
Care Coordinators will send referrals electronically (by email) to the Manager. Once received the Manager will arrange a full needs assessment.
Facilities
