Overview
Layton Court and Strauss Court is purpose built accommodation for people with Mental Health issues to live independently in the community. It comprises of 23 high quality spacious self contained Flats.
The service provides personalised support to vulnerable adults with a range of mental health issues who require regular support at a time convenient to them to enable them to maintain their mental health, and physical health and to be able to acquire the skills to live independently.
Our friendly and well trained staff work with each person to develop tailored support based on their needs and aspirations.
The length of stay in this service will be up to 2 years, but can be flexible to provide longer term support if identified as an assessed need.
Staff are on site 24 hours a day, 7 days a week to provide individually tailored support to the customers.
What we offer
- To develop and maintain independent living skills to be able to move on within two years of placement.
To deliver person centred support promoting individuals to make informed choices and achieve their aspirations.
Ensure safety and wellbeing by working closely with specialist services to support individuals to work through their recovery.
Areas supported
The areas of support provided but not limited to:
- Daily living skills (cooking / cleaning)
- Budgeting, paying bills and preventing debt
- applying for welfare benefits such as Housing Benefit, Council tax benefit, PIP
- Education, training, voluntary work & employment
- Health care appointments
- Leisure & culture
- Accessing the community
- Building relationships
- Repairs
- Health, safety & wellbeing
- Drug & alcohol support / signposting
Customer groups
Vulnerable adults with a severe & enduring mental health issue.
Referrals
Referrals come from the Community Mental Health team Care Co-ordinators in the London Borough of Merton Via the Mental Health Accommodation panel who meet monthly to consider suitability of applicants for referral to Ability’s Layton Court/Strauss service.
The London Borough of Merton Mental Health Accommodation panel will send referrals electronically to the Manager. Once received the Manager will arrange a needs assessment.
Facilities
The accommodation is within walking distance of local shops and amenities with good public transport links.
